I can honestly say I am so happy with the service and care I received

I made an appointment to meet Mr. Harley to discuss what could be done with my nose. I’ve been unhappy with the appearance of my nose for years (I had a steep bridge).

Mr. Harley examined my nose and took pictures from different angles. He showed me photos of before and after work, he’s carried out on other patients in or a similar situation and started to talk about what could be done with mine.

Mr. Harley understood what I wanted to be done and showed me on his computer roughly what it could look like. He was very honest and professional throughout which made me feel confident I was making the right decision.

Later that day I received an email from Mr. Harleys secretory Linda with my quotation, which included a full break down of all the costs. I personally thought the price was reasonable for the type of operation I was expecting to have. As soon as I agreed to go ahead, Mr. Harley wanted to see me again Incase I had any other questions and to make sure I was 100% happy.

On the morning of my operation, I checked in at reception and got shown to my room. The nurses introduced themselves and done some tests (all the nurses are extremely helpful and kind). Mr. Harley came and saw me make sure I was ok and took some more photos.

After my operation I was looked after very well by everyone - nurses and healthcare assistants kept checking on me and offering me something to drink/eat. Mr. Harley saw me later that day to see how I was and explained how the operation went. After a comfortable night, it was almost time for me to go home. The nurses and Mr. Harley done some final tests and gave me some medication to take home - they explained what I should do if I had any problems and to call anytime. After a week, I had my splint and stitches removed. Mr. Harley was pleased with the result and so was I 100%! He explained it takes around a year for the nose to completely settle down which is understandable.

Overall I can honestly say I am so happy with the service and care I received. All the staff are friendly, professional and always wanting to help. Mr. Harley is very professional, honest and caring - I would recommend him to anyone as he takes great pride in his work and looks after his patients.

Excess skin can gather on the upper arms and this is a combination of ageing which increases skin laxity and weight loss after being overweight.  Women are most usually affected by this problem and may be self-conscious about their appearance in armless tops and dresses or in swimwear.  It may also be difficult to fit into the arms of blouses or jackets.

What is an Arm Lift?

Arm Lift reshapes the upper arms by selectively removing the excess skin and restoring a slimmer contour to the upper arm.  The skin is carefully assessed and measured prior to the surgery in order to achieve this.  An extended arm lift deals with skin folds which extend below the arm pit onto the side of the chest wall.

Does the surgery require a general anaesthetic or hospitalisation?

Arm lift surgery is done under general anaesthetic, as a day case or a one-night-stay procedure.

How long is the procedure?

Arm-lift normally takes approximately 2 hours to complete depending on the complexity of the surgery. 

How long is the recovery time?

Moderate pain / discomfort is expected for 3 – 5 days. Patients can go back to their normal daily activities after 2 weeks, but avoid strenuous exercise / activity for 6 weeks. The arm will feel slightly tight after surgery.  It will be necessary to wear a support garment or tubigrip for the first 6 weeks, day and night and the second 6 weeks during the day. There will be bruising and swelling and some tightness which will mostly resolve within 6 weeks but it will take at least 3 months to completely subside.

You may have decided to change the size of your breast implants or to have them removed altogether.  Alternatively the shape or texture of one or both breasts may have changed with time.  This is typically due to a membrane which forms around the breast implant, tightening and thickening with time - it is known as a ‘capsule’.  As the capsule tightens it causes the shape of the breast to become more rounded; wrinkles can sometimes be seen or felt and sometimes there is pain also.  Capsule tightening and the shape change which results occurs in approximately 1-2% of implants per year.  Change in body and breast shape following pregnancy or weight loss may also have contributed to this.

What does the operation involve?

Usually, your previous breast implant scar is reopened and sometimes it may be made a little longer.  The ‘capsule’ is removed in part or as a whole (‘capsulectomy’) or it may be left behind but loosened with some internal incisions (‘capsulotomy).  The old implant is also removed and typically this is exchanged for a new implant of the same or of a different size.  The old implant can be returned if it is in good condition.  Some women choose not to have the breast implant replaced at all. 

A small plastic tube (‘drain’) is positioned in the breast cavity and it is connected externally to a vacuum bottle.  The drain is left in position for approximately 48 hours and then removed.

  Ordinarily you will go home with the tube in place and return to the hospital to have it removed on the second or thrid day after operation.  Sometimes the operation will be combined with an uplift of the breasts (‘mastopexy’) especially if a smaller implant is chosen.  Mastopexy usually involves an incision around the nipple and may also require a vertical incision from the nipple to the fold and a longer incision in the fold.  This adds some further complexity and time to the operation.

Does Breast Implant Surgery require a general anaesthetic or hospitalisation?

Breast Implant Surgery is usually a 1-night-stay or a day-case operation which is carried out under general anaesthetic.

What are my options for the type of implants?

Various types of Breast Implants are available and the optimum implant for you will depend on your goals and existing size and shape. Breast implant types are Silicone or Saline filled, round or teardrop shaped and can be low, medium, high or extra high profile in order to achieve the best aesthetic natural result.  There are differences in the outer layer of implants also.  In the last few years, special light-weight implants ‘B-lite’ have become available - these are a more expensive option.

Where will the implants be placed in Breast Augmentation Surgery?

Breast implants can usually be placed directly behind the breast tissue.   If the breast tissue is thin over the upper part of the breast then it may be preferable to place the implant more deeply, under the pectoral muscle. In either position, what you will see and feel is your own breast tissue. 

The goal is to achieve the optimum natural appearance of your breasts.

How long is the recovery time?

Most patients report that their surgery was much less painful than they expected. Moderate pain / discomfort is expected for 3 – 5 days. You may return to your normal daily activities the next day if you feel ready and comfortable to do so.  You should however avoid strenuous exercise / activity for 6 weeks. It will be necessary to wear a support bra for the first 3 weeks, day and night and the second 3 weeks during the day. The majority of swelling will resolve in 2 – 3 weeks but it will take up to 3 months for the breast to return to normal feel.

How would I be helped by breast reduction?

You may find that your breasts are too big.  This could be for reasons of body image perhaps if you have a slight or slim figure which is out of keeping with the size of the breasts.  Regardless of your overall body shape, large and heavy breasts can cause pains around the neck and shoulders, affect posture or cause rubbing and discomfort from bra straps.  The breast crease can be sweaty and even develop rashes or sores.  

If you are sporty you may find that a big bust interferes with your activities.  From time to time women have jobs or pastimes which may be difficult due to physical requirements or special clothing.  Other patients find that large breasts are a source of unwanted attention and comment. All of these problems are successfully addressed by breast reduction; women have high rates of satisfaction following breast reduction surgery and this is supported by medical research into quality of life after this procedure. 

How is the operation done?

One of the most important aspects of breast reduction surgery is your consultation.  Mr Harley will listen carefully to your concerns and discuss look you want to achieve.

The operation itself is done while you are asleep (general anaesthetic).  It takes 2-3 hours and you can go home on the day of surgery or the day following it. 

Breast reduction removes skin and breast tissue usually from the underneath and sides of the breast and reshapes the remaining breast tissue to give natural and more youthful appearance.  This is carefully measured and planned at your consultation and again immediately prior to surgery.  The remaining skin and tissue is reshaped around the nipple and the resulting breast is smaller, lighter and uplifted.

What is Brow Lift?

Browlift is a procedure to elevate the resting position of the eyebrows.  This also has some effect on the upper eyelids when there is some excess skin.  It is often helpful to combine brow lift with a procedure to remove skin from the eyelids (upper eye bag removal; upper blepharoplasty).

How would I be helped by browlift?

Browlift is intended to rejuvenate and refresh the appearance of the eyes.  It may also improve your field of vision if this has become reduced by a heavy brow or upper eyelid.  Frown lines on the forehead and between the brows are also typically smoother after brow lift.

With advancing years the tissues of the face tend to descend with gravity.  Around the eyes, the brow moves to a position just below the upper rim of the eye socket.  Typically this is combined with an excess bag or fold of skin on the upper eyelid.  The result of this may be a hooded, tired, mournful or even fierce appearance to the eyes.  Sometimes a heavy brow can obscure peripheral vision and patients may be aware of heaviness, tiredness or headaches associated with straining the forehead muscles to keep the eyes fully open.

Introduction

Upper eyelids

When an extra fold of skin develops on the upper eyelids the eyes look ‘hooded’ and may have a tired, sad or ageing appearance.  This tends to happen with the passage of time but it can also be significant in younger adults.  Sometimes there is so much extra skin that it rests on the eyelashes and it can obscure peripheral vision.  

Lower eyelids

Extra skin also develops on the lower eyelid.  In addition, there is a small pad of fat which sits under the eyeball and this may spread forwards.  Wrinkles and bags are present in the lower eyelid as a result of these changes.  This makes eyes look puffy and tired.  Again, this is age related but it may be seen in younger adults.  

What is blepharoplasty?

This is a plastic surgical procedure which refreshes the eyes.  Blepharoplasty treats hooding and heaviness of the upper eyelids and removes bags and wrinkles in the lower eyelid.   As a result the eyelids look younger, less tired and puffy.  You may also feel more comfortable and improve peripheral vision by having excess eyelid folds removed.

What is pinnaplasty?

The medical term for the external ear is the ‘pinna’.  A pinna-plasty alters the shape of the external ear.  In almost all cases this involves altering prominent ear so that it sits closer to the side of the head.  People talk about ‘pinning’ ears…..no pins are involved!  The shape and position of the ear is corrected by scoring the cartilage in the ear and / or holding it in a new position with stitches.  A headband is worn full time for a week to protect the corrected ears.

Why do ears stick out?

This is simply a variation between humans.  The cartilage of the pinna is a complex of folds and recesses which forms during the first few months of foetal development.  In most cases the prominent ear will have quite a deep bowl ‘concha’ immediately next to the ear hole.   Additionally, the principal fold in the mid and upper section of the pinna has a very open angle or it is deficient altogether.  Prominent ears can be a source of ridicule and self-consciousness and for this reason patients frequently ask for the ears to be corrected.

How are prominent ears corrected?

In adults, this surgery is usually done as awake surgery with local anaesthetic.  Children would routinely have general anaesthetic and some adult patients may prefer to have general anaesthetic also.

What is Rhinoplasty?

Rhinoplasty is also known as a ‘nose-job’.  It is an operation which changes the shape of the nose to give a more aesthetically balanced appearance or perhaps to correct an established deformity.  There are a combination of changes which can be made to improve the appearance of the nose which include reshaping the bridge of the nose (usually reducing a hump), refining the tip of the nose, improving the projection of the tip, shortening the nose or narrowing wide nostrils.  The aim is to shape the nose so that it is more in keeping with the desired appearance of the patient and also in balance with the proportions of the face.

What does the surgery involve?

Rhinoplasty is typically carried out under general anaesthetic and you can go home on the day of surgery.  Sometimes a single night in hospital is advised.  The shape of the nose is determined by a framework of cartilages and bones.  Small cuts are made inside the nostrils to access the framework and reshape it.  Occasionally it is also necessary to have a small scar at the base of the nostrils or across the central strut (‘columella’) of the nose or tiny access incisions on the side of the nose.  The operation takes about 90 minutes. At the end of the operation a small splint and some tapes will be positioned and packs are inserted into the nostrils.  This is done to support the new shape of the nose while it sets.  Some cases are more complex and may require internal access to the central strut deep inside the nose ‘septum’ which is necessary to correct shape, airflow or to obtain cartilage grafts to assist with shaping the nose.  This is known as septo-rhinoplasty.

Rhinoplasty is also known as a ‘nose-job’.  It is an operation which changes the shape of the nose to give a more aesthetically balanced appearance or perhaps to correct an established deformity.  There are a combination of changes which can be made to improve the appearance of the nose which include reshaping the bridge of the nose (usually reducing a hump), refining the tip of the nose, improving the projection of the tip, shortening the nose or narrowing wide nostrils.  The aim is to shape the nose so that it is more in keeping with the desired appearance of the patient and also in balance with the proportions of the face.

What does the surgery involve?

Rhinoplasty is typically carried out under general anaesthetic and you can go home on the day of surgery.  Sometimes a single night in hospital is advised.  The shape of the nose is determined by a framework of cartilages and bones.  Small cuts are made inside the nostrils to access the framework and reshape it.  Occasionally it is also necessary to have a small scar at the base of the nostrils or across the central strut (‘columella’) of the nose or tiny access incisions on the side of the nose.  The operation takes about 90 minutes. At the end of the operation a small splint and some tapes will be positioned and packs are inserted into the nostrils.  This is done to support the new shape of the nose while it sets.  Some cases are more complex and may require internal access to the central strut deep inside the nose ‘septum’ which is necessary to correct shape, airflow or to obtain cartilage grafts to assist with shaping the nose.  This is known as septo-rhinoplasty.

Why do I need a thigh lift?

Excess skin can gather between the upper thighs and this is a combination of ageing which increases skin laxity and weight loss after being overweight.  Women are most usually affected by this problem and may be self-conscious about their appearance in shorts or swimwear.  There may be wetness or soreness from rubbing skin.

What is an Thigh Lift?

Thigh Lift reshapes the inner thighs by selectively removing the excess skin and restoring a slimmer contour to the thigh.  The skin is carefully assessed and measured prior to the surgery in order to achieve this.  Liposuction may be used in addition.

Oliver Harley MBBS, BSc, MBA, FRCSEd(Plastic Surgery)

Oliver Harley is a fully qualified, GMC registered specialist Plastic Surgeon.   He is also a member of The British Association of Plastic Reconstructive and Aesthetic Surgeons (BAPRAS), and The British Society for Surgery of the Hand (BSSH).  Mr Harley specialises both in cosmetic surgery and hand surgery.

After qualifying in medicine in 1998 at Guy’s and St Thomas’ Hospitals in London he has gained extensive experience in the field of Plastic and Reconstructive Surgery since 2002.   Mr Harley completed his higher surgical training at St Thomas’ Hospital (London), St Georges Hospital (London) and Queen Victoria Hospital (East Grinstead) and was awarded FRCS (Plastic Surgery) in 2009.  

He further developed his expertise by undertaking specialist training fellowships in Cosmetic Surgery (at McIndoe Surgical Centre, East Grinstead 2010) and in Hand and Wrist Surgery (at Wrightington Hospital, Lancashire and Erasmus MC University Hospital, Rotterdam 2011-2012).  He has worked as an NHS consultant in plastic surgery in Plymouth, the John Radcliffe and the Nuffield Orthopaedic Hospitals in Oxford, and at the Queen Victoria Hospital in East Grinstead.
