Breast Implants
One of the most frequent interventions in our Clinic is the augmention mammoplasty or the enlargement of breasts. This is an intervention by which the volume of the breast is augmented and its shape is improved.The surgery is performed: * with women who have naturally small breasts * after the atrophy after giving birth * for correction of asymmetric breasts * as reconstructive surgery after surgical removal of cancerIn this intervention an implant is inserted in the area behind the gland (retroglandular position) or behind the muscle (retromuscular position). The incisions are located in the area of the lower rim of the areola (darkly pigmented area around the nipple), in the area of the ridge between the breast and the chest or in the axilary pit. The wounds are finished by fine surgical technique resulting in minimally visible scars. The scars are red at first but they become paler. Each approach has its advantages and disadvantages. When you come for consultations you will be informed on them.Types of implantsIn our Clinic we mainly use the implants of the renowned company Mentor. They may be of different shape and profile. The external layer is made of silicone and they are filled with physiological solution or cohesive silicone of the latest generation. Before the surgery we will explain to you all the advantages and disadvantages of different types of implants.
 
Breast Lift
Sagging breasts of normal size or atrophic ones may be primarily as such, e.g. with younger girls or, which is more usual, with women after giving birth. The procedure is similar to that of breast reduction (link), but here only the skin is tightened and the nipple and areola are moved to a higher position. Gland and fat tissues are not reduced or, if they are, it is done at the lower pole of the breast and very little.
 
Breast Reduction
Large, heavy and often hanging breasts do not only represent an aesthetic problem influencing the proportions of the female body, they also pose a medical problem which may result in back pain and wrong posture with nerve compression problems in the upper extremities. The principle of the procedure is to remove the excess skin, gland and fat tissue and move the nipple and areola to a higher, aesthetically acceptable position. Classic incision is in the shape of an anchor, while more recently the preferable incision is the so called short vertical scar technique with no horizontal scar in the inframamary fold. Large quantities of gland and fat tissue may be removed with aesthetically pleasing results. We also use the technique of autoaugmentation.The pendulous part of the gland and fat tissue is repositioned and fixed under the strip of pect. Major muscle. The technique gives more permanent result and better shaping of the breast. In the first few months the scars are reddish and somewhat wrinkled but in time they lose the redness and become less visible. We recommend treating the scars with silicone gel or silicone membranes. Recently we have started using PDL - Vbeam laser to treat the red,imatured scars. The results are promising. After the surgery the patient should wear a supportive bra. Bathing is possible after the stitches are removed, sunbathing after a month.

Blepharoplastyis one of the most frequent procedures for rejuvenating in aesthetic surgery. Excess skin with bags on upper and lower eyelids is usually a sign of premature aging in the periorbital area. A simple intervention can significantly rejuvenate this area. Excess skin and fat are removed from the upper lids while on the lower lids mainly fat is removed and in rare cases part of the muscle and skin. Excess skin and wrinkles in the area of lower lids are resolved by pulsing CO2 or Erbium laser. Laser beam erases the wrinkles and tightens the skin due to the process of collagen and elastin regrouping. The use of laser enables faster recovery after the surgery with less bruising and swelling. The surgery is performed in local anesthesia with sedation or in general anesthesia.
 
Facelift
The first signs of aging are shown by appearance of wrinkles, loss of the skin elasticity, loss of the lower jaw edge, appearance of deep creases between the nose and the mouth and by the creases on the neck. The process does not take place only on the skin but also in the deeper structures such as the muscles and support skin structures (ligaments, fascias etc). The principle of the operation is in lifting of the sagging subcutaneous structures and bringing them to their original natural position. In this way the sagging tissue above the naso- labial folds (between the nose and the upper lip) is returned to the area of cheek bones where it belonged and the sagging mass of fat in the area of lower jaw is lifted to the area of cheeks. Neck muscles are stretched upwards and backwards. The skin is adapted over these new deep structures, without exaggerated tightness which gives the face a natural look. In order to access these structures and perform the lifting, the incision is placed in the natural creases around the ear and in the hair line. After healing the scar is barely visible and does not need to be camouflaged by hair. Deep lifting (SMAS) is tightening and lifting of above- mentioned deeper structures and is the golden standard of modern aesthetic surgery of the face. Gynecomastia
The name ginecomasty comes from the Greek word which denominates the female breasts that can be found on men. About 30% of the male population has this anomaly of unknown cause. Usually it is the combination of overgrowing of the fat and gland tissue which provides volume to the breast and makes it look female. The principle of the procedure is to remove the fat and gland tissue by liposuction. Sometimes surgical removal of gland tissue by a semicircular incision around the nipple is also required besides the liposuction. After the intervention the patient wears a special bodice like an elastic vest. Removal of fat and gland tissue decreases the size of the breasts with the skin retracting to the new layer through the pectoral muscle.
 
Liposuction
Liposuction, hydro- liposuction and lipo- sculpturing are terms for surgical method which are used to sculpt the body by suction of fat from some parts of the body. Typical accumulations which have negative impact on woman’s body are the abdominal wall, thighs, hips, area of riding trousers, internal part of the knee, upper arm and chin. Sometimes also liposuction of the area around the ankles is performed. Liposuction is not a method of weight loss and is therefore not recommended for persons whose weight is significantly above ideal. The principle of the procedure is to first introduce liquid to facilitate the removal of fat tissue and diminish the bleeding. After that hollow tubes are used to remove excess fat in symmetric layers on both sides. After the procedure the skin is retracted to the new layer. In order to facilitate the retraction, the patient wears a bodice for a couple of weeks. We have recently started to use the so called oscillating hollow tubes which enable the insertion of much thinner tubes so the recovery is much faster and the bruising is less expressed. Liposuction with male patients Accumulation of fat tissue may diminish the harmonious body image with men, particularly in the area of the waist, lower abdomen (love handles) and in the area of the chest (ginecomasty) (link to ginecomasty). The principle of the procedure as well as the post- operational course are the same as in liposuction for women.
 
Otoplasty
Correction of prominent ears is one of the most frequent surgeries performed on children, but sometimes adults also choose this intervention. The growth and development of the earlobe is complete at the age of four so after that time it is possible to surgically bring the earlobes closer to the head. It is advisable to perform the surgery before children start school in order to prevent them from being teased by his/her peers, which is usual at that age and may have serious consequences in the later psychological formation of the personality. The principle of the surgery is to model the cartilage of the ear and to bring the earlobe closer to the head. The intervention lasts from 30 minutes to one hour. It may be done in local or general anesthesia. The bandages remain in place for one week and the stitches are removed after the bandages are taken off. The incision is barely visible because it is located in the crease between the ear and the head. It is recommended to wear a bandage over the ears after the stitches are removed, for about one month.

Lip Augmentation
Lip augmentation is performed with younger persons who have a smaller volume or somewhat older persons who experienced atrophy of tissue in the area of the mouth. There are two basic groups of materials (fillers) for lip augmentation. One group belongs to the so called permanent fillers, the other in semi permanent or resorbing fillers. In Croatia, Aquamid is used as permanent filler and among others the most usual one is hiyalouronic acid ([Treatment name removed]). In our Clinic we often use the autologous fat to augment the lips. The choice of method depends on the patients preference and objective status of the lips which will be augmented.
